We are committed to make available to you a personal physician who assist with your healthcare needs and coordinates your care, including the medical office, hospital, clinics, testing facilities, and other places where you receive healthcare. 

 

You may request care and advice by calling 936-291-2116 during our office hours, which are 7:45am to 5:00pm Monday - Friday.  If you need care or advice after office hours, call 936-291-2116 and speak with the on-call physician. 

 

We expect you to provide us with your medical history, as well as information about any care you obtain outside of our practice, to include your current medications, recent test results, visits to other doctors and healthcare providers, hospitalizations, and emergency department visits.
